INITIAL HEALTH EVALUATION

 

The Initial Health Evaluation is the required first step for all new patients and returning patients that have not visited the doctor within a year.  This is the most efficient way for the doctor to obtain sufficient information to make an informed decision about the next steps in the patients program.  There are two main components to it:

Relevant Documentation

All of the relevant documents must be submitted to Miramas Health Clinic at least 3 days prior to the scheduled appointment.  If they have not been received by then the appointment will be rescheduled (with a $25 administration).

Once the forms have been received then Dr. Kershaw will review and study the documents so that she is prepared and the phone conversation is more productive. 

Phone Consultation

The patient will call the clinic (780 423 6633) at the appointed time and the conversation should last between 30-45 minutes.  Please be prepared and ready at your scheduled phone consultation time.  Have handy any questions or notes you made and also some blank paper and pen.

The purpose of this phone call is for Dr. Kershaw to completely understand your case and history as it relates to your current health issues.  This is also a time for you, the patient, to ask questions that are related to your health history and understanding of it.  Questions that relate to the Miramas Health Clinic, the programs, and therapies will be diverted to the website. 

At the end of the conversation the doctor will explain what she feels are the most appropriate next steps.  This may be any combination of the following several possible outcomes:
